2.  Fax the completed form to ATTN: Curt Massey, Rph, CGP at 1-800-764-1184

3.  Wiithin 24 hours or receipt of the form, you will be emailed a confirmation and applicable passwords to access the member section of the website.

4.  Educational materials can be downloaded (Adobe pdf format) by following links from the Home page of the SHCA website.  The appropriate password(s) is required for each download.
